Key Responsibilities

  • Evaluate patients' physical abilities and develop comprehensive treatment plans
  • Provide individualized therapy sessions, including exercises, stretches, and other interventions
  • Document patient progress, adjust treatment plans as needed, and communicate with referral sources and other healthcare providers
  • Collaborate with other therapists, nurses, and medical professionals to ensure seamless care

Requirements

  • Licensed or eligible for licensure by the State of Illinois under the Illinois Physical Therapy Practice Act
  • General knowledge and therapy skills to perform evaluations and treatments across various conditions and age groups
  • Certification in CPR and other service line specific competencies
  • Ability to work independently and collaboratively as part of a multidisciplinary team
