We are seeking a highly skilled and compassionate Clinical Social Worker to join our team at Stony Brook Medicine. As a key member of our healthcare team, you will provide comprehensive care to our patients, focusing on their mental health and well-being.
Key Responsibilities- Conduct thorough clinical assessments to identify patients' mental health needs and develop personalized care plans.
- Collaborate with our medical and administrative staff to coordinate comprehensive care for our patients.
- Provide supportive counseling and education to patients and their families, addressing mental health concerns and promoting healthy coping mechanisms.
- Facilitate referrals to community resources and external providers, ensuring seamless care coordination.
- Document patient interactions and care plans in our electronic medical record system.
- Master's degree in psychology, counseling, or a related clinical field.
- Valid NYS MSW license.
- Two years of full-time case management experience or clinical social work experience.
- Experience working with patients with mental health conditions, particularly post-traumatic stress, depression, anxiety disorders, and substance abuse disorders.
- Additional years of full-time case management experience or clinical social work experience.
- Experience working with the first responder population.
- Experience administering clinical psychiatric diagnostic interviews and assessments.
- Case management or medical social work experience specific to assisting cancer patients or patients with cognitive decline/dementia.
