Key Responsibilities:
- Deliver personalized care services tailored to the individual needs of our clients.
- Promote the well-being of program participants through attentive support.
- Act as the primary coordinator of care during scheduled shifts, including weekends and holidays.
- Adhere to all regulatory standards, policies, and procedures established by CommUnityCare.
Qualifications:
- Education: High School diploma or GED is preferred.
- Experience: Minimum of six months in providing personal care for elderly or disabled individuals in settings such as group homes, assisted living, or long-term care facilities, with experience in dementia care being a plus.
Work Schedule:
- Day Shift: 6:30 AM - 3:00 PM, totaling 36 hours per week.
- Availability for every other weekend and holiday is required.
