POSITION OVERVIEW
Mainstream Living is dedicated to providing services that enhance opportunities, foster success, and realize aspirations.
The primary objective of this role is to oversee programs related to medication administration, as well as physical and mental health, ensuring effective service coordination for individuals supported by Mainstream Living.
The Wellness Coordinator also serves as a vital resource and facilitator for the organization, individuals served, their families, and guardians.All staff members are expected to cultivate a safe and inviting, home-like atmosphere that inspires hope and supports individuals in achieving their personal goals and recovery journeys.
REQUIREMENTS AND COMPETENCIES
Education:
A High School Diploma or equivalent (HiSet/GED) is required, along with certification as a Certified Medication Aide (CMA) or Certified Nursing Assistant (CNA), or similar training/education preferred.
Experience: A minimum of 5 years in health service coordination or related experience is preferred. Candidates should possess advanced knowledge gained through a combination of professional experience and certification or training. Experience in behavioral health and/or working with individuals with intellectual disabilities is preferred.
Driving:
This role necessitates a valid Driver's License, legal driving privileges, and the ability to qualify as an insurable driver according to Mainstream Living's underwriter.
Other Certifications:
As required to maintain licensure (if applicable)
Computer Skills:
Proficiency in computer applications including data entry, word processing, Google Docs, and basic spreadsheet skills is required.
Post-Hire Requirements:
Candidates must demonstrate competencies to complete internal training, including training to administer medication. Completion of medication aide certification is required within 6 months.

The Wellness Coordinator position requires meticulous attention to detail and exceptional organizational skills.The individual in this role should excel in clear communication and relationship-building through active listening, adapting their approach to create a welcoming and inclusive environment.
This individual must demonstrate sound judgment in crisis situations.The Wellness Coordinator must be flexible and collaborate with the team to devise strategies and systems that effectively address challenges with resourcefulness.
They must be able to objectively assess situations and determine appropriate actions.
The Wellness Coordinator is expected to serve as a role model for both individuals and staff, while also exemplifying the company's values by maintaining positive and professional relationships with individuals, guardians, case managers, providers, and other stakeholders regarding direct services and specific needs of the individuals served.
KEY RESPONSIBILITIES
This position is classified as an essential healthcare worker role, and in emergencies, may be required to work closely with individuals to ensure their safety.
Direct Service Responsibilities:
Under the guidance of the Health Coordinator, RN, this position will engage in approximately 10-20 hours per week of direct involvement in member care coordination, including but not limited to:
Participating in provider appointments
Attending interdisciplinary team meetings.
Assessing and evaluating the needs of individuals served in terms of supervision and safety.
Ensuring that health needs are met and that rehabilitative services are consistently provided to individuals served, addressing both physical and psychological health.
Providing guidance and support regarding nutritional needs as outlined in individual supervision plans.
Determining and communicating the appropriateness of services for individual needs and making referrals to other services as necessary.
Collaborating directly with guardians, families, and partner agencies/providers as needed.
Advocating for individuals (regarding their care, needs, and desires) and promoting overall well-being in a manner that is satisfactory to the individual and their team, while complying with agency policies and regulatory standards.
Administrative and Support Responsibilities:
Assisting in the development, modification, maintenance, and auditing of medication systems (including compliance with relevant policies and regulations regarding documentation and administration).
Assisting in monitoring compliance with Corrective Action Plans and their applicability to other members and/or programs.
Assisting in monitoring member sites and living environments to ensure safety, implementing systems to address safety concerns, trends, or hazards. Participating in the Safety Committee as needed.
Maintaining knowledge of Department of Inspection and Appeals rules and regulations to provide consultation and assistance to staff and members.
Assisting in maintaining healthcare and emergency supplies.
Performing other duties as assigned.
Compliance with Policies and Procedures:
All employees are required to adhere to Mainstream Living's policies and procedures, including but not limited to:
Personnel Policies and Policy manuals.
Attending and working shifts as scheduled, following Mainstream Living's Attendance Policies for time off, on-call duties, and attendance at training/staff meetings.
Adhering to medication and medical policies and procedures, including administration, record-keeping, identification of medication, therapeutic intent, adverse reactions, and self-administration of medication.
Complying with health and safety practices as per Mainstream Living's policies, including participation in emergency procedures, drills, and proper infection control.
Keeping the supervisor informed of critical and non-routine events, ensuring communication is constructive, effective, and timely.
WORK ENVIRONMENT
The characteristics of the work environment described are representative of those an employee may encounter while performing the essential functions of this role.
Physical Requirements:
While performing this job, employees must be able to meet the physical demands necessary to fulfill essential functions and meet the needs of individuals in their home environments.
Employees will be required to lift 15-25 pounds frequently.Depending on the needs of individuals and in certain work locations, employees may need to assist with ambulation, lifting, and transferring individuals, typically using adaptive equipment such as gait belts, walkers, wheelchairs, and mechanical lifts.
As such, there may be instances where employees must lift up to 50 pounds and occasionally up to 100 pounds when working with individuals requiring ambulation assistance.
OSHA CLASSIFICATION
This position is classified as Classification I concerning jobs that routinely involve a potential for contact with blood, body fluids, tissues, or potential spills or splashes.
Generally, the schedule aligns with business hours, with some evenings and weekends required.
40 hours/week.
