About the Role
We are seeking an experienced Family Medicine Specialist - Clinical Lead to lead the development of our new primary care model, focusing on caring for patients 55+ with two or more chronic conditions. This role will involve designing and implementing efficient care processes, leveraging virtual and in-person visits, remote patient monitoring, and interdisciplinary collaboration.
Responsibilities
- Leverage clinical expertise to manage complex patient cases and develop targeted care plans.
- Collaborate with cross-functional teams to design and implement care pathways, ensuring seamless transitions and optimized patient outcomes.
- Promote a culture of compassion, innovation, and growth within the team, fostering open discussion and feedback.
- Act as a mentor to Advanced Practice Providers (APPs) and the care team, providing guidance and support.
- Contribute to the advancement of our primary care model, driving continuous improvement and excellence.
Qualifications
- Board certified or board eligible in Family Medicine or a related specialty.
- Licensed to practice medicine in Illinois and eligible for Medicare, Medicaid, and other federal health programs.
- Excellent communication and leadership skills, with a proven track record in clinical management and team collaboration.
Benefits
- Competitive salary range: $250,000 - $300,000 per year.
- 4 weeks paid vacation and 6 paid holidays.
