Responsibilities:

  • Administer medications and treatments to residents in accordance with their individualized care plans
  • Document medication administration and resident care in a timely and accurate manner
  • Conduct Service and Health Updates as directed by the Resident Care Director
  • Participate in the development and implementation of Individualized Service Plans (ISPs)
  • Collaborate with the care team to ensure resident-centered care and promote a positive living environment
  • Maintain accurate and up-to-date records of resident care and medication administration
  • Report any concerns or issues related to resident care or medication administration to the Resident Care Director
  • Participate in ongoing education and training to stay current with best practices in medication management and resident care

Requirements:

  • Current LPN licensure in the state of employment
  • Experience working with seniors and in a long-term care setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic computer skills and proficiency in electronic medical records
  • Ability to maintain confidentiality and handle sensitive information
