About the Job

We are seeking a skilled Licensed Practical Nurse (LPN) Supervisor to join our team at Ontario Center for Rehabilitation and Healthcare.

Key Responsibilities:
  • Conduct regular facility rounds to ensure a safe and efficient environment.
  • Monitor the assignments of aides to optimize workflow and productivity.
  • Supervise medication assistance times to ensure accurate recording and follow-up as needed.
  • Intervene in resident conflicts and behavior issues, facilitating timely communication and resolution.
  • Ensure all incident reports are completed and summarized for the EHP Coordinator/Administrator.
  • Attend to late admissions and re-admissions, including paperwork and medication changes.
  • Communicate shift activity through 24-hour reports for review by the Enriched Housing Program Coordinator/Administrator.
  • Facilitate communication for environmental situations, contacting the EHP Coordinator/Administrator as needed and recording details for review.
Requirements:
  • Ability to work collaboratively as a team member.
  • Valid state LPN license and good standing with the State Registry.
