Job Title: Charge Nurse

At Constructive Partnerships Unlimited, we are seeking a highly skilled and experienced Charge Nurse to join our team. As a Charge Nurse, you will play a critical role in ensuring the highest quality care is provided to our patients.

Job Summary:

The Charge Nurse will be responsible for overseeing the provision of services provided to patients by clinic physicians, clinicians, RNs, LPNs, and DCCs. This includes ensuring that all health needs are addressed in an appropriate and efficient manner.

Key Responsibilities:
  • Supervise RNs and LPNs and provide oversight and instructions to the nursing staff and direct care staff in the OPWWD certified programs.
  • Monitor and evaluate staff performance related to delegated tasks.
  • Assure the accurate implementation of the health care plan by the nursing staff.
  • Evaluate the effectiveness of care plans in meeting all outcomes, goals, and objectives.
  • Communicate outcomes to the people we serve, families, caregivers, and service providers.
  • Provide updated reports and ensure proper documentation by staff.
  • Monitor compliance with established policies and procedures.
  • Monitor scheduling and completion of medical appointments.
  • Monitor and ensure completion of documentation as per nursing standards and agency policy.
  • Monitor activities in all clinical areas to ensure quality services are provided.
  • Complete monthly schedules for nurses and ensure daily shifts are adequately staffed.
  • Monitor facility medication storage and ensure adequate medical supplies are available.
  • Collaborate with the physician on the disposition of unscheduled/priority appointment requests.
  • Conduct monthly nurses meetings and provide verbal and written guidance as needed.
  • Provide in-service training for 24-hour nurses as required.
  • Develop and update the people we serve profile with the Residential Coordinator or other service providers.
  • Assign nursing staff to perform health care treatments/procedures.
  • Supervise the preparation of packages for the Surrogate Court, Consumer Advisory Board, MIPS, and MOLST.
  • Attend Surrogate Court proceedings as necessary.
  • Attend annual and semi-annual ISP meetings/conferences.
  • Maintain a safe, aseptic, and therapeutic residential setting environment.
Requirements:
  • NYS licenses Registered Nurse.
  • Minimum three years nursing experience with one year in supervision.
  • C.P.R. certification.
  • Extensive knowledge of pertinent health resources.
  • Excellent oral and written communication skills.
  • Able to work flexible hours and adhere to the agency's corporate compliance program.
