Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Community Options, Inc. in Pottsville, PA.
Job Summary:The LPN will deliver high-quality care to individuals with disabilities in a community setting, utilizing the nursing process to assess, plan, implement, and evaluate care.
Responsibilities:- Provide treatment and healthcare procedures ordered by a physician and required by professional practice or law.
- Ensure individuals attend medical appointments per their Individual Support Plan (ISP).
- Advise supervisor of all concerns and issues related to nursing services.
- Create and distribute medical/health reports detailing general health, healthcare goals, and physician recommendations.
- Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team.
- Train employees on medical needs of assigned individuals.
- Monitor health and medical conditions of individuals and refer for medical services as needed.
- Monitor medication use through medication administration logs.
- Ensure health service supplies are available and replenished as needed.
- Ensure program documentation and billable records are completed accurately and timely.
- Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families.
- Participate with individuals in community, social, and recreational activities.
- Participate in circles of support for personal futures planning.
- Follow strict OSHA procedures regarding blood-borne pathogens.
- Bachelor's Degree or equivalent experience in nursing field.
- Minimum of one year of experience providing nursing services to individuals with disabilities.
- Valid nursing license.
- Valid driver's license with a satisfactory driving record.
- Complete all state and agency required training per state guidelines.
- Ability to calculate figures related to medication dosages.
- Understanding and commitment to community-based support for persons with disabilities.
- Ability to exercise good judgment and remain calm in crisis situations.
- Ability to establish a comfortable and supportive relationship with an individual receiving supports.
- Proficient with Microsoft Office.
- Excellent verbal and written communication skills.
- Ability to effectively present facts and recommendations.
- Ability to effectively communicate with program participants, third-party agencies, staff, and members of the community.
