About the Role

We are seeking a skilled Speech Language Pathologist to join our team at CenterWell Primary Care. As a Speech Language Pathologist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Evaluate and provide speech/language pathology services to patients in their homes or facilities
  • Develop and review treatment plans to ensure optimal patient outcomes
  • Administer and interpret diagnostic tests to identify and treat speech, language, and hearing disorders
  • Provide instruction and training to patients and their families on alternative communication systems and strategies
  • Collaborate with healthcare teams to ensure comprehensive care and effective discharge planning
Requirements
  • Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology
  • Minimum of six months experience as a speech therapist/speech language pathologist
  • Home health experience a plus
  • Current and unrestricted license
  • Current CPR certification
