About the Role:

We seek a skilled Nurse Practitioner to provide high-quality medical care to seniors and patients with complex needs in various settings, including assisted living facilities and private homes.

Key Responsibilities:

  • Deliver evidence-based medical care by assessing patient needs, developing treatment plans, and evaluating their progress.
  • Examine and treat chronic and acute illnesses, minor injuries, and other health issues.
  • Order and interpret laboratory and diagnostic testing as needed.
  • Collaborate with healthcare professionals to ensure comprehensive care and maintain accurate patient records.
  • Provide referrals to other healthcare practitioners when necessary.

Requirements:

  • Active state NP license
  • National Board Certification
  • State DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience, with at least two years of nurse practitioner experience preferred
  • Strong communication and interpersonal skills
  • Excellent time management and organizational skills
  • Proficiency in electronic medical record systems and Microsoft Office applications
