Intake Specialist

We are seeking a highly motivated and compassionate ADRC Specialist to join our team at the Council on Aging of Southwestern Ohio. As a key member of our organization, you will play a vital role in providing exceptional support to older adults and individuals with disabilities in our community.
Key Responsibilities:
Assist callers from our 5-county area in navigating the aging and disability network, providing information and resources to help them make informed decisions about their long-term care options.
Complete telephone and computerized screenings to determine eligibility for COA-administered services and refer individuals to community agencies as needed.
Work closely with COA staff to communicate consumer-related issues and provide support to callers and their caregivers.
Maintain current knowledge on COA programs, Medicare, Medicaid, and long-term care topics to provide accurate assistance to callers.
Adhere to COA's confidentiality policy, consumer rights, and HIPAA protocols when interacting with individuals and their information.
Assist consumers in selecting the best Medicare D options by providing education and information.
Provide training and education to the community and staff on aging and disability resources and issues as needed.
Obtain and maintain certification for OSHIIP, AIRS, or other recommended programs.
Maximize the use of technology and reporting to enhance and improve operations.
Requirements:
Bachelor's degree in a social service field or equivalent experience in a relevant field.
AIRS certification or ability to obtain within one year of employment.
At least one year of experience in healthcare, community outreach, community resources, medical social work, geriatrics, or providing information about long-term care options.
Good working knowledge and understanding of issues related to the elderly and disabled population.
Tolerant, empathetic, and excellent interviewing skills.
Knowledge of community resources in Southwestern Ohio.
Excellent oral and written communication skills.
Ability to adapt to frequent change and build cooperation among team members.
A valid driver's license and reliable transportation.
Excellent computer skills, including Windows, MS Word, MS Excel, data entry, and the ability to learn new software applications.
Ability to travel within the state of Ohio for training.
