At Mass General Brigham, we're seeking an experienced Case Management Nurse Specialist to join our team. This is a full-time opportunity for a skilled Registered Nurse (RN) with a passion for delivering exceptional care and supporting patients in need.

Job Summary:

  • We are looking for a dedicated RN Case Manager who can provide skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice.
  • The successful candidate will be responsible for providing services utilizing nursing theory and processes, communicating/documents observations and assessments, and promoting and maintaining the health of individuals through direct care, teaching, counseling, and appropriate rehabilitative measures.
  • This role reports to the VNA Clinical Team Manager and operates within established organizational and departmental policies and procedures.

Responsibilities:

  • Provide skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice.
  • Communicate and document observations and assessments to ensure comprehensive patient care.
  • Promote and maintain the health of individuals through direct care, teaching, counseling, and appropriate rehabilitative measures.
  • Maintain accurate and up-to-date records of patient care and progress.

Requirements:

  • Graduate of an approved, accredited school of professional nursing required; BSN preferred.
  • Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
  • Minimum of one (1) year Medical/Surgical nursing experience required; home care/hospice experience preferred.
  • Demonstrated excellent verbal and written communication skills and strong interpersonal skills required.
  • Demonstrated organizational and time management skills required.
