Key Responsibilities
  • Provide comprehensive case management services to patients and their families, ensuring seamless transitions and optimal health outcomes.
  • Collaborate with healthcare professionals, including physicians, therapists, and social workers, to develop and implement individualized care plans.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address complex health issues.
  • Coordinate and facilitate patient education, support, and resources to promote health literacy and self-management.
  • Monitor patient progress, adjust care plans as needed, and ensure timely interventions to prevent hospitalizations and readmissions.
  • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and progress.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices and policies.
Requirements
  • Graduate from an accredited school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN.
  • Licensed to practice as a registered nurse in the state of agency operation.
  • Must possess and maintain valid CPR certification while employed in a clinical role.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
