Job Summary

The successful candidate will provide a combination of office and community-based nursing services, working closely with clients and other team members to coordinate care and meet the physical and behavioral health needs of our population.

Key Responsibilities
  • Provide integrated care for clients, including nursing knowledge of physical and behavioral health needs

Requirements
