Supplemental Health Care is seeking a skilled and dedicated LPN to join our team as a Home Health Nurse. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our team to ensure seamless care coordination.
Job Responsibilities:- Provide comprehensive home health care services to patients, including wound care, medication management, and vital sign monitoring.
- Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to meet their unique requirements.
- Collaborate with our interdisciplinary team to ensure continuity of care, including physicians, therapists, and other healthcare professionals.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and progress.
- Document patient progress, including medical history, treatment plans, and outcomes, in accordance with regulatory requirements.
- Current Georgia Nursing License
- Minimum of 1 year recent home health nursing experience (private duty does not qualify)
- Electronic Charting experience
- Medicare Certified Agency Experience
