RN Home Healthcare Case Manager

We are seeking a skilled and compassionate RN Home Healthcare Case Manager to join our team at PeaceHealth. As a key member of our home healthcare team, you will be responsible for providing direct skilled nursing care and coordination/management of patients admitted to home care/clinic services.
Responsibilities- Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services.
- Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.
- Develop, coordinate, modify and manage written individualized plan of care, as ordered by the primary physician, to meet patient's long and short-term medical goals and appropriate/timely home health outcomes.
- Coordinate care, collaborate and communicate with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.
- Perform discharge planning appropriately towards patient specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
- Provide accurate, complete and timely clinical documentation in compliance with regulatory, quality and Agency guidelines.
- Bachelor's Degree in Nursing required; Bachelor of Science in Nursing preferred.
- 2 years of acute clinical or home health/hospice nursing experience required; 1 year of home health experience preferred.
- Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient's needs.
- Registered Nurse in state of practice required; Drivers License and proof of auto insurance required; Basic Life Support required upon hire.
This position requires the ability to lift patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices. The ability to move around area with frequent sitting and bending/stooping/squatting/reaching/kneeling is also required.
