**Travel Nursing Opportunity for Clinical Case Manager RN in Sacramento, California**

About the Role:

We are seeking an experienced Clinical Case Manager RN to join our team on a 13-week assignment in Sacramento, California. As a Clinical Case Manager RN, you will collaborate with the healthcare team to assess, plan, facilitate, and coordinate care for individual patients.

Key Responsibilities:

  • Oversee the effective utilization of services and resources within and outside of facilities
  • Communicate with patients, families, other healthcare professionals, and social workers to promote patient safety, quality of care, and cost-effective outcomes
  • Develop and implement care plans that meet the unique needs of each patient

Requirements:

To be successful in this role, you must have at least 1 year of recent acute care experience in a specialty as a Registered Nurse (RN). You must also possess an active RN license and excellent communication and organizational skills.
