Role of the Inpatient Pharmacist:
The Inpatient Pharmacist is responsible for ensuring the safe and effective use of medications for patients. This includes reviewing, processing, and monitoring physician orders and patient medication profiles.
As a vital member of the healthcare team, the Staff Pharmacist will collaborate with other healthcare professionals, fulfilling duties in alignment with the hospital's mission and values.
Position Details:
- Location: Logansport, IN
- Licensure: Must possess an active and valid IN State Pharmacist License in good standing
- Education: Pharm.D. or B.S. in Pharmacy
- Experience: Minimum of 2 years in a relevant setting
- Work Schedule: Monday - Friday, 7:00 AM - 5:00 PM
- Compensation: Starting at $70.00
- Background Requirements: Willingness to undergo a background check and health/drug screening
Key Responsibilities:
- Ensure the appropriate and safe use of drug therapy; assess patient drug regimens for suitability, toxicity, effectiveness, interactions, adverse reactions, and allergies.
- Collaborate with healthcare professionals to identify and resolve medication-related issues.
- Review physician order entries for accuracy and appropriateness, addressing any discrepancies with relevant personnel.
- Recommend individualized pharmacotherapy plans for patients.
- Provide drug consultations, including antibiotic dosing, as requested by physicians.
- Participate in the Medication Usage Evaluation program.
- Act as a resource for drug information for patients and healthcare professionals.
- Document interventions and activities as mandated by legal and departmental policies.
- Assist with medication reconciliation processes.
- Conduct research on drug information and fulfill dispensing responsibilities.
- Maintain all required legal and departmental records.
- Verify the work of pharmacy technicians and students prior to dispensing medications.
- Prepare and supervise the preparation of sterile compounds, including I.V. syringes and admixtures.
- Stay updated on drugs, drug therapy, and relevant regulations.
- Cover all areas of Pharmacy Services and responsibilities, including IV admixture certification.
- Attend departmental meetings and continuing education sessions.
- Adhere to all departmental, hospital, state, and federal regulations and policies.
