Job SummaryWe are seeking a skilled Full-Time or PRN Certified Physical Therapist to join our team in the Suffolk area. As a physical therapist with Personal Touch Home Care, you will play a vital role in implementing standards of care for physical therapy services and delivering high-quality patient care.
Key Responsibilities:- Assess and evaluate therapeutic/rehabilitative/functional status and participate in developing the total plan of care.
- Direct physical therapy treatment and instruct patients and families/caregivers on the use and care of therapeutic appliances.
- Determine priority needs for physical therapy and report to physicians patient reactions to treatment or changes in condition.
- Initiate physical therapy programs and instruct other personnel and/or family/caregiver members in certain phases of physical therapy.
- Supervise physical therapy assistants, home health aides/personal care aides when such personnel are participating in patient rehabilitation regimes.
- CURRENTLY licensed as an Physical Therapist in the State.
- PREFERRED two (2) years of experience as an occupational therapist. Home Health experience preferred.
- DEMONSTRATES good verbal and written communication and organization skills.
- STRONG computer skills; familiarity with Allscripts/NetSmart preferred.

Pay Information$38.00 per hour
Qualifications- Master's (Preferred)
- Physical Therapy License (Required for State of VA)
- CPR Certification (Required)
