Job OverviewThis part-time opportunity allows you to leverage your expertise and compassion to make a direct impact on the health and well-being of others.
You will have flexible scheduling while working in various community environments in Cleveland, Ohio.
As a Nurse Practitioner, you will be rewarded with competitive compensation and a mileage reimbursement plan.
Key Responsibilities- Conduct adult and geriatric assessments, medical history, diagnosis, and treatment, as well as provide health education and referrals.
- Work collaboratively with physicians, case managers, social workers, family members, and other healthcare professionals as needed.
- Collaborate with Primary Care Physicians (PCPs) on patient education and follow-up.
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
- Masters degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
- Ability to travel and possess a valid state driver's license and adequate automobile insurance coverage.
- Travel required to meet patients in their homes.
