Job Summary

Community Concepts is seeking a dedicated Family Support Specialist to work with families with young children in Western Oxford County. This role involves partnering with families to support parenting skills, monitor children's developmental progress, and collaborate with families to establish a source of continuous, accessible, coordinated developmental, health, and dental care.

Key Responsibilities:

  • Support families in their homes to promote healthy development and well-being
  • Monitor children's progress and provide guidance to families
  • Collaborate with families to access community resources and services
  • Document and maintain accurate records of family interactions and progress

Requirements:

  • Minimum of a home-based CDA credential or equivalent
  • Minimum of two years of home visiting experience
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
