Home Health Physical Therapist Position in Lewes, Delaware

We are seeking a skilled Home Health Physical Therapist to join our team in Lewes, Delaware. This is a fantastic opportunity to make a meaningful difference in the lives of our patients and their families.
Job SummaryThe successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, they will develop and implement a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate their progress.
Key Responsibilities- Evaluate the patient's functional status and physical therapy needs
- Assist the physician in developing the therapy plan of care
- Observe and report the patient's response to treatment and any changes in their condition to the supervising nurse and/or physician
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice
- Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals
To be considered for this role, you must have:
- Current Physical Therapy licensure in the state of Delaware
- Current CPR certification
- A current driver's license and vehicle insurance, as well as access to a dependable vehicle
We offer a competitive salary range of $90,000 - $120,000 per year, based on experience. Additionally, we provide:
