Job Title: Financial Case Manager

Rochester, NY

Job Summary:

We are seeking a skilled Financial Case Manager to join our team at Lifespan. As a Financial Case Manager, you will play a critical role in assisting older adults in managing their financial affairs and maintaining their personal independence.

Key Responsibilities:

  • Assess and investigate the financial situation of clients
  • Determine and plan appropriate actions to assist clients in becoming financially secure
  • Provide full bill paying and budgeting services to ensure long-term security
  • Assist with financial decision-making
  • Provide referrals to other community resources to help clients remain independent
  • Maintain and manage all records, case notes, billing, reports, and metrics as required
  • Advocate with financial, creditor, and other institutions on behalf of clients
  • Appropriately bill clients for services rendered

Requirements:

  • Associate in Applied Sciences required; Bachelor's degree in Business, Finance, Human Services, or equivalent preferred
  • 2 years' banking or financial services experience required; must have strong financial acumen and a working knowledge of community resources
  • Expertise in Medicaid and other benefit programs preferred; prior experience working with and advocating for older adults preferred
