Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Gaudenzia. As an LPN, you will play a vital role in providing high-quality patient care, working collaboratively with our multidisciplinary team to ensure the best possible outcomes for our clients.

Responsibilities
  • Perform initial nursing assessments for clients within 24 hours of admission, adhering to established program policies and state regulations.
  • Arrange appointments with healthcare providers to secure physical examinations, health screenings, and supplemental tests as needed.
  • Administer medications and supervise self-administered medications, ensuring compliance with regulations and agency policies.
  • Maintain accurate records of medical supplies and inventory, adhering to governing regulations and agency policies.
  • Counsel clients on nutrition, healthcare, self-care, and symptom management.
  • Ensure clients have appropriate medical coverage for safety, adequate care, and provider compliance.
  • Implement health seminars on topics such as HIV/AIDS, nutrition, medication education, and infectious diseases.
  • Participate in treatment and multidisciplinary team meetings as directed.
  • Implement and ensure adherence to infection control policies and practices.
  • Provide education to clients on health, nutrition, safety, and medication compliance.
  • Maintain LPN licensure with good standing.
Working Conditions

This position requires working weekends at least once a month, including one off-shift per week, to ensure program coverage. May be required to work shifts outside of assigned shifts during critical staffing shortages. The minimum holiday requirement is 1 major summer holiday and 1 major winter holiday.

Qualifications

To be considered for this role, you must be a graduate of an accredited school of practical nursing with a valid LPN or RN license in good standing. New graduates and newly licensed LPNs and RNs are welcome to apply, as direct job experience is not a requirement for this position.
