Key Responsibilities- Develop and implement evidence-based wound care plans to improve patient outcomes
- Collaborate with interdisciplinary teams to coordinate patient care and promote seamless transitions
- Provide education and training to patients and families on wound care and management
- Conduct regular assessments and evaluations to monitor patient progress and adjust treatment plans as needed
- Participate in quality improvement initiatives to enhance wound care services and patient satisfaction
- Current licensure as a Registered Nurse in the State of New York
- Minimum of 5 years of experience in wound care or a related field
- Strong clinical skills and knowledge of wound care principles and practices
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks
