Job Summary

We're seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Nurse. As a Home Health Nurse, you'll provide comprehensive nursing care to clients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Provide comprehensive health and psychosocial evaluations to clients, identifying their unique needs and developing individualized care plans.
  • Monitor clients' conditions, providing ongoing care and support to ensure optimal health outcomes.
  • Coordinate services with other healthcare professionals, including physicians, therapists, and social workers.
  • Teach and train clients and their families on self-care techniques and disease management.
Requirements
  • Must be a Registered Nurse licensed by the state.
  • Must have one or more years of experience in home health care or a facility setting.
  • Must have knowledge of Medicare and Medicaid guidelines.
  • Must have a working knowledge of home health care and the principles and techniques of professional nursing.
