We are seeking a highly motivated and compassionate Peer Navigator to join our team at Detroit Recovery Project. As a Peer Navigator, you will play a critical role in providing direct support to individuals receiving services for treatment.
Key Responsibilities- Provide individualized assessments of identified families eligible for Peer Navigator Pilot.
- Assist families in connecting to community resources, including home visitation programs.
- Provide families with a warm handoff to the home visiting service that best fits their needs.
- Provide scheduled follow-up for families to identify any additional needed support.
- Assess for and provide facilitation of the completion of a plan of safe care on every identified family.
- Participate in staff training and development.
- Attend monthly Peer Navigator Group Mental Health Consultations.
- Advocate on behalf of persons with behavioral health problems to protect their rights and reduce associated stigma.
- Input data directly into iPad/tablet during services.
- Develop and maintain written documentation (assessments, service plans, progress notes, monthly reports, and other programmatic documentation).
- High School Diploma or GED is required.
- Associate's degree in guidance and counseling, psychology, social work, vocational rehabilitation, business or public health administration or a related field is preferred.
- Certified Peer Recovery Coach (CPRC) and/or Certified Peer Recovery Mentor (CPRM) preferred.
- First Aid/CPR Certification.
- Past or present recipient of mental health or substance abuse services with at least 3 years of long-term uninterrupted sobriety.
- Past recipient of home visiting services preferred.
- A valid and unrestricted State of Michigan Driver's License, proof of insurance, and reliable transportation.
- Proficiency with Microsoft Office Suite.
- Experience with Electronic Health/Medical Records Systems.
- A collaborative environment with Clinical Site Leaders and regular peer review.
