We are seeking a highly skilled Substance Use Navigator to join our team at Rely Health. As a key member of our healthcare team, you will play a vital role in supporting patients with substance use disorders and connecting them with the resources they need to achieve sobriety.
Key Responsibilities- Assess patient readiness for treatment and link them to care
- Utilize motivational interviewing techniques to communicate with patients in a respectful and non-judgmental manner
- Maintain up-to-date information about substance use disorders and treatment options
- Provide liaison services to Addiction Medicine and outpatient clinics
- Communicate effectively with patients, family members, and caregivers
- High school diploma or equivalent
- Understanding of substance use disorders and medication-assisted treatment
- Nonjudgmental and culturally competent approach to assisting patients
- Respect for patient confidentiality and privacy
- Healthcare experience and proficiency in medical terminology
