Job Title: Transitioning Youth Specialist

The Arc Southern Maryland is seeking a skilled Transitioning Youth Specialist to join our team. As a key member of our staff, you will play a vital role in supporting youth with intellectual and developmental disabilities (IDD) as they transition from school-based services into adult life.

Job Purpose:

The Transitioning Youth Specialist will ensure the provision of tailored services to help individuals achieve successful outcomes in areas such as employment, higher education, community involvement, and independent living.

Key Responsibilities:
  • Develop and implement individualized transition plans for youth with disabilities, working closely with students, families, educators, and external service providers.
  • Act as a liaison between families, schools, government agencies, and community organizations to ensure the seamless delivery of services, including job coaching, post-secondary education options, and vocational training.
  • Advocate for youth with disabilities to ensure access to necessary resources, supports, and opportunities for personal growth.
  • Organize and conduct workshops for youth and their families on topics related to employment, life skills, and accessing community resources.
  • Collaborate with the team to develop innovative programs and services that address the unique needs of transitioning youth.
  • Identify and secure resources and services that support successful transitions, including funding, partnerships, and new initiatives.
  • Maintain up-to-date case notes, transition plans, and progress reports in compliance with organization and funding requirements.
Essential Duties and Responsibilities:
  • Employment: Assist individual(s) to meet his or her employment goals, providing supports, and building on individual strengths and capabilities.
  • Volunteer and Recreational Skills & Supports: Provides on-site skill instructions and overall job training while encouraging development of natural support systems.
  • Community and Service Networking: Develops and/or assists with the development of perspective employer relationships, and the school system.
  • Participant Empowerment: Assists individual(s) to plan, make informed choices, follow through on responsibilities and speak on his/her/their own behalf.
  • Communication: Uses effective, sensitive communication skills to build rapport and channels of communication by recognizing and adapting to the range of individual(s) communication styles.
  • Facilitation of Services: Maintains collaborative professional relationships with individual(s) and all support team members (including family/friends), follows ethical standards of practice (e.g., confidentiality, informed consent, etc.), and recognizes his or her own personal limitations.
  • Crisis Intervention: Identifies crisis situations, evaluates or defuses the situation, and contacts necessary supports.
  • Documentation: Maintains accurate records and submits records to appropriate sources in a timely fashion.
Qualifications:
  • Bachelor's degree preferred.
  • Minimum of 2 years of experience working with youth with intellectual and developmental disabilities, ideally in a transition or case management role.
  • Knowledge of local, state, and federal services and benefits available to youth with disabilities (e.g., DORS, Social Security, Medicaid).
  • Strong organizational and time-management skills, with an ability to manage multiple cases and tasks simultaneously.
  • Excellent communication skills, both written and verbal.
  • Ability to work collaboratively with diverse stakeholders, including families, schools, and community partners.
  • Familiarity with the Americans with Disabilities Act (ADA), Individuals with Disabilities Education Act (IDEA), and other relevant legislation.
  • Experience with person-centered planning and individualized support plans.
Job Knowledge, Skills, and Abilities:
  • Possesses a proficiency in computer-based software, e.g., Excel, Word.
  • Demonstrated understanding of and willingness to participate in a team environment.
  • Ability and willingness to learn and to apply techniques of crisis management.
  • Demonstrated ability to analyze information and come to appropriate resolution.
  • Demonstrated ability to collaborate with all employees, employers, and community members.
  • Possesses a valid driving license with a good driving record, and access to a safe vehicle (must pass agency-sponsored inspection) for transporting people receiving services.
  • Completion of an agency-approved advanced employment special certification and/or completion of agency-approved applied behavior specialist coursework.
  • Must be able to work a varied schedule in the tri-county area, as assigned.
  • Possesses the ability to pass the criminal background check mandated by the State of Maryland.
