Clinical Pharmacist I

Job Summary:
We are seeking a highly skilled Clinical Pharmacist to join our team at CHRISTUS Health. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and ensuring the safe and effective use of medications.
Key Responsibilities:
- Meet the expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
- Prepare and dispense drug orders per physician request according to established policies, procedures, and protocols.
- Interpret drug orders (verbal and written) and transcribe/verify into computerized patient medication record accurately.
- Compound and dispense pharmaceuticals, including non-sterile and sterile products, chemotherapy, and parenteral nutrition products accurately.
- Issue controlled substances to patient care areas and maintain records as required by law and institutional policies.
- Provide services efficiently and in a timely fashion.
- Maintain current pharmacist license and ensure applicable CE records and licensure are maintained in department files.
- Attend staff meetings and complete all competency/skills assessment requirements.
- Supervise and direct pharmacy support personnel, verifying the daily activities of pharmacy technicians.
- Provide ambulatory pharmaceutical care services, managing patient drug therapy, anticoagulation management, patient education, and preparing and/or overseeing the preparation and dispensing of medications.
- Perform duties in clinical areas, such as anticoagulation clinics, intensive medical home, transition of care, discharge medication reconciliation, etc.
- Ensure safe, appropriate, cost-effective drug therapies for patients according to established policies, procedures, and protocols.
- Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
- Assist with pharmacokinetics consult service and renal drug dosing per hospital protocol.
- Read, extract, and interpret information in patient medical records accurately.
- Detect and report suspected adverse drug reactions accurately and in a timely manner.
- Sustain the hospital drug formulary, minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
- Provide clinical consultation and clarification to practitioners as appropriate.
- Provide accurate, adequate, and timely drug information to the hospital's professional staff.
- Provide drug education to patients and their families per institutional protocol.
- Participate in quality improvement and medication use review activities of the department, collecting data, conducting monitors and inspections, and maintaining logs, records, and other documentation as assigned.
- Conduct designated interventions as defined in department Clinical Intervention activities (IV to PO, Renal Dosing, Pharmacokinetic dosing support, Anticoagulation).
- Participate in antibiotic stewardship, opioid stewardship, and shortage management activities as assigned.
- Attend interdisciplinary rounds when assigned.
- Provide services efficiently and in a timely fashion.
Requirements:
Doctor of Pharmacy or BS Pharmacy degree required. Hospital experience preferred. Pharmacy registration in the State of practice. Must have pharmacy State licensure within 60 days of start date. BLS or ACLS and PALS (pediatric practice) within 180 days of start date.
