Job Title: Physical Therapist - Home Health Care Professional

At Cross Country Allied, we are seeking a skilled Physical Therapist to join our team as a Home Health Care Professional. As a Home Health Physical Therapist, you will provide high-quality physical therapy services to patients in their homes, helping them regain their health and mobility.

Job Summary:

We are looking for a dedicated and compassionate Physical Therapist to work with our patients in a home health setting. As a Home Health Physical Therapist, you will assess patients' mobility, strength, and function, develop treatment plans, maintain records, and provide patient education. Your expertise will play a critical role in helping patients develop strength, reduce pain, and improve function.

Responsibilities:
  • Assess patients' mobility, strength, and function in their homes
  • Develop and implement individualized treatment plans to meet patients' needs
  • Maintain accurate and detailed records of patient progress
  • Provide patient education on proper techniques and exercises to promote recovery
  • Collaborate with other healthcare professionals to ensure comprehensive care
Requirements:
  • At least 1 year of recent experience in a home health setting
  • Degree in relevant field (e.g., Physical Therapy, Occupational Therapy)
  • BLS Certification (AHA)
  • Current state/compact license
