We are seeking a highly skilled Registered Dietician to join our team at Elderwood. As a key member of our healthcare team, you will be responsible for providing nutrition assessments, developing individualized care plans, and educating residents and their families on healthy eating habits.
Responsibilities- Conduct comprehensive nutrition assessments for adult and geriatric populations in a long-term care and sub-acute rehabilitation setting
- Develop, implement, and monitor individualized nutrition care plans based on current best practices
- Provide nutrition counseling and education to residents and their families
- Maintain accurate and up-to-date resident nutrition information in our computer system
- Conduct routine audits and monitor performance improvement initiatives related to nutrition and food service
- Collaborate with the multidisciplinary medical team to ensure comprehensive care
- Act as a nutrition consultant and preceptor for Dietetic students and interns
- Bachelor of Science Degree or higher in Nutrition/Dietetics from an accredited educational program
- Successful completion of an internship or equivalent clinical management practice
- Active and ongoing Registration with the Commission on Dietetic Registration
- Preferred qualifications include long-term care experience and computer skills in nutritional software programs
