Key Responsibilities

Your role will involve:

  • Upholding the mission, vision, and values of Vera Whole Health
  • Providing evidence-based primary care to 10 to 15 patients daily, emphasizing preventive care and effective management of chronic and acute conditions
  • Collaborating with patients and your care team, including an onsite Whole Health Coach, to develop personalized care plans that reflect each patient's unique needs
  • Ensuring comprehensive care aligned with the Vera medical home model, which includes gathering detailed health histories, conducting necessary physical examinations, performing diagnostic tests, and prescribing medications through onsite lab and pharmacy services
  • Assessing the healthcare needs of the patient population and engaging team members to encourage participation in annual health evaluations and health promotion initiatives
  • Providing clinical leadership by actively participating in team meetings, maintaining clear communication, prioritizing tasks, driving process improvements, and supporting change initiatives
  • Meeting customer contractual requirements and expectations in collaboration with your care team
  • Participating in rotating schedules with other providers for after-hours coverage
  • Engaging in organizational activities and committees to enhance Vera's processes and programs
  • Achieving objectives by improving the health status of the eligible population and meeting benchmarks for clinical outcomes, engagement, and satisfaction
Qualifications

The ideal candidate will possess:

  • MD or DO degree
  • Completion of a Family Medicine or Internal Medicine Residency, with a preference for Family Medicine
  • Current Board Certification by ABMS (ABFM/ABIM) or AOA
  • A minimum of three years of experience in primary care
  • A valid and unrestricted medical license along with a valid DEA number
  • Experience in team-based care
  • Proficiency in EMR systems, preferably NextGen
  • Knowledge of coding and documentation, including Medicare Advantage and HCC
  • Familiarity with Google Workspace

Compensation: $238k-$350k annual salary (national average, may vary based on markets)
