Job Title: Infusion RN

Centers Plan for Healthy Living is seeking a skilled Infusion RN to join our team. As an Infusion RN, you will be responsible for providing high-quality patient care and developing care plans that meet the unique needs of each patient.

Responsibilities:
  • Conduct thorough patient assessments and create personalized care plans
  • Coordinate resources and services for assigned caseloads
  • Maintain accurate and detailed documentation of patient care
Requirements:
  • Active and unrestricted NYS RN license
  • Previous experience in infusion and home health care
  • Excellent communication and interpersonal skills
  • Valid Driver's License and reliable transportation
Preferred Qualifications:
  • Proficiency in Microsoft Office Suite
  • Bilingualism is a plus
Compensation:
  • $130.00 per visit
