Job Title: RN - Case Manager

American Medical Staffing is seeking a skilled RN - Case Manager to join our team of healthcare professionals. As a RN - Case Manager, you will play a critical role in ensuring the highest level of patient care and satisfaction.

Job Summary:

We are seeking a highly motivated and experienced RN - Case Manager to join our team. The successful candidate will be responsible for managing patient care, coordinating with healthcare teams, and ensuring seamless transitions between care settings.

Responsibilities:

  • Manage patient care plans, ensuring alignment with individual patient needs and goals
  • Coordinate with healthcare teams, including physicians, nurses, and other healthcare professionals
  • Ensure timely and effective communication with patients, families, and healthcare teams
  • Monitor patient progress, identifying areas for improvement and implementing changes as needed
  • Collaborate with healthcare teams to develop and implement care plans that meet individual patient needs
  • Ensure compliance with regulatory requirements and industry standards

Requirements:

  • Current RN licensure in the state of Virginia
  • Minimum 2 years of experience as a RN - Case Manager
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Proficiency in electronic medical records and other healthcare software
