Job Summary:

The Home Health Nurse will provide high-quality nursing care to patients in their homes, working under the direction of the Director of Nursing. This role requires strong communication skills, the ability to work autonomously, and a commitment to delivering exceptional patient care.

Responsibilities:
  • Conduct timely start of care visits, initial assessments, routine visits, and re-evaluations as assigned
  • Develop, implement, and maintain nursing care plans for assigned patients
  • Initiate preventative and rehabilitative nursing procedures as needed
  • Observe and report symptoms, reactions to treatments, and changes in patient condition
  • Counsel patients and families on meeting nursing and related needs
  • Supervise and delegate tasks to LPNs, CNAs, and HHAs as needed
  • Complete accurate and timely documentation
  • Participate in on-call rotation and attend in-service events
Requirements:
  • Bachelor's degree in Nursing from an accredited School of Nursing or equivalent
  • 1-3 years' experience in Home Health preferred
  • Current licensure as a registered nurse in the state of Florida
  • Current State of Florida driver's license with driving record acceptable to Alivia Care insurance carrier
  • Current Auto Insurance coverage
  • Current CPR certification
  • Level II AHCA Background Screening
Preferred Qualifications:
  • Bi-lingual Spanish
  • Computer proficiency with Microsoft Office
Medical Clearance:

Employees must have medical clearance for communicable diseases before direct patient contact. Provide Covid-19 vaccination card and current immunizations, including TB, Hepatitis, and Flu.
