We are seeking a highly skilled Registered Nurse to join our team as a Behavioral Health Home Health Specialist. As a key member of our team, you will provide high-quality patient care and support to individuals in their homes, promoting their physical and emotional well-being.
Responsibilities- Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Conduct regular home visits to assess patient progress, provide education, and address any concerns or issues that may arise.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
- One year of recent nursing experience, recommended to be within the last 3 years, in an acute treatment unit in a psychiatric hospital, psychiatric home care, a psychiatric Partial Hospitalization Program (PHP), or another out-patient psychiatric service.
- Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role.
- Ability to travel to all business locations.
