Job Summary

We are seeking a skilled Healthcare Liaison Specialist to join our team at Fortcom Health Services. The successful candidate will be responsible for promoting our agency and fostering community relations, while applying their unique understanding of the experience, language, or culture of the populations we serve.

Key Responsibilities

  • Assist in promoting the agency and fostering community relations
  • Apply unique understanding of the experience, language, or culture of the populations serve
  • Assist in the evaluation of community needs and services to meet with identified needs
  • Act as liaison with member/caregiver and the agency
  • Establish and maintain effective channels of communication
  • Provide culturally appropriate health education, information, support, and counseling
  • Participate in the development, implementation, and ongoing review of the AFC plan of care
  • Conduct on-site visits with the member at the qualified setting
  • Assist with obtaining information and accessing other health-care and community services and making referrals to appropriate service providers if the member requires services other than those provided by the AFC provider in conjunction with RN

Requirements

  • At least one year of experience working with elders or adults with disabilities
  • Ability to apply unique understanding of the experience, language, or culture of the populations serve
  • Must live in Massachusetts
