Job Title: LPN Supervisor - Resident Care Coordinator

At Weinberg Campus, we are seeking a skilled LPN Supervisor to join our team. As a key member of our nursing staff, you will be responsible for planning and implementing the health needs of our residents in the Assisted Living Residence, Enhanced Assisted Living Residence, and Special Needs Assisted Living Residence.

**Responsibilities:**

* Assess the individual needs of our residents to ensure their physical, mental, social, recreational, and religious needs are being met.
* Plan and provide for the overall comfort, safety, and well-being of our residents.
* Coordinate on-going nursing care and evaluate health and emergency concerns.
* Maintain accurate records according to facility policies and New York State Health department regulations and guidelines.

**Requirements:**

* 18 years of age or older
* Empathetic and sensitive towards the geriatric population
* Ability to maintain a leadership role
* Ability to speak, read, and write English and speak the predominant language of our residents
* Ability to lift greater than 30 pounds
* Ability to walk/stand for extended periods of time

**Working Conditions:**

* High chance of exposure to human bodily fluids
* Must maintain the privacy, dignity, and rights of our individual residents
