Key Responsibilities- Visit patients in their homes and provide nursing care, including administering medications, wound care, and vital sign monitoring.
- Collaborate with RNs and other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- Current licensure as a Licensed Practical Nurse in the state of employment.
- Valid state driver's license and reliable transportation.
- Ability to drive in all types of weather conditions.
- Dependable and punctual, with excellent time management skills.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
