At LHC Group, we are seeking a skilled Home Health Physical Therapist to join our team in South Orange, New Jersey. This is an ongoing opportunity that requires schedule flexibility to ensure we provide care to patients as needed.

Job Summary

The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. They will work with the physician to develop a treatment plan, perform interventions aimed at improving and enhancing patient well-being, and evaluate patient progress.

Key Responsibilities
  1. Evaluate patient functional status, body systems, and physical therapy needs, consulting with physicians to develop therapy plans of care.
  2. Provide services within scope of practice, following state laws governing physical therapy, in accordance with therapy plans of care and coordination with healthcare teams.
  3. Observe and report patient responses to treatment and changes in condition to nurse supervisors and physicians.
  4. Treat patients to relieve pain, restore function, and maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
  5. Instruct participating healthcare team members, patients, and family/caregivers regarding therapy plans of care, functional limitations, and progress toward goals.
  6. Visit patients throughout service areas and nearby LHC agencies as needed, completing progress notes according to Plan of Treatment and submitting them per LHC policy.
  7. Complete patient evaluations, develop PT plans of care within state-specific guidelines, and report outcomes, goals, and projected frequency of care to nurse supervisors within 24 hours of evaluation.
  8. Participate in staff conferences and committees as requested by the agency, supervising home health aides and providing supervision of PTAs per LHC policy and state regulation.
  9. Require schedule flexibility, including weekend rotation and potential availability on observed holidays, to meet patient demands.
  10. Prepare written discharge summaries and/or summary reports on patients' conditions every 60 days.
  11. Provide in-service education as requested by agency leadership, adhering to LHC Therapy Standards of Practice in provision of services to all patients.
  12. Maintain up-to-date knowledge and skills through education and enhance clinical abilities to provide current standards of care for the profession and maintain state-issued licensure.
Requirements and Qualifications
  • Current Physical Therapy licensure in New Jersey.
  • Current CPR certification.
  • A valid driver's license and vehicle insurance, with access to a dependable vehicle.
Estimated Salary Range

We offer a competitive salary range of $90,000 to $120,000 per year, commensurate with experience. Our comprehensive benefits package includes holiday pay, medical, dental, vision benefits, a 401(k) retirement plan, continuing education opportunities, and employee assistance programs.
