Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team in a PRN/Part Time capacity. As a Home Risk Assessment Specialist, you will play a critical role in improving the quality of care for our patients by conducting comprehensive health assessments in the comfort of their own homes.
Key Responsibilities:
- Conduct thorough adult and geriatric assessments, medical history reviews, and diagnosis and treatment planning
- Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy outcomes
- Work in a variety of settings, including home visits, skilled facilities, and telehealth
Requirements:
- Master's degree in Nursing or equivalent experience and licensure
- Current RN and NP licensure with prescription authority
- Board certification in a Matrix-approved specialty
- 1+ year of experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
