Job Description

This Case Manager/ QMHP position requires a strong understanding of social work principles and practices, as well as excellent communication and interpersonal skills. As a key member of our team, you will engage with clients in Harris and surrounding areas, providing case management and psychotherapeutic services to address their unique challenges and needs.

Responsibilities:
  • Assess clients' situations through thorough interviews and reviews of personal history.
  • Develop and execute individualized treatment plans to meet clients' specific needs.
  • Determine suitable milestones to measure client progress and adjust treatment plans accordingly.
  • Communicate client updates and progress to relevant stakeholders, including family members, healthcare professionals, and insurance providers.
  • Facilitate referrals to other healthcare professionals and programs as necessary.
  • Maintain accurate and up-to-date client documentation.
Qualifications:
  • Bachelor's Degree in Social Work, Psychology, or related field.
  • At least 2 years of experience in social work, counseling, or related fields.
  • Excellent written and verbal communication skills.
  • Ability to build rapport with clients and families.
  • Willingness to obtain ANSA and CANS certification.
  • Flexibility to work on the road/in-person as needed.
