Patient Advocate Job Description

At Inbound Health, we prioritize quality and safety in our Hospital-at-Home care program, serving over 6,000 patients across 350+ primary diagnoses.

As a Clinical Liaison, you will serve as the face of our program, liaising between IH and hospital partners and patients. You will provide patient care coordination for Hospital-at-Home to ensure continuity of care for program patients.

Key Responsibilities Include:

  • Drive growth objectives by developing outreach strategies and implementing plans to achieve business goals.
  • Identify new opportunities to increase utilization and patients who require IP level of care in accordance with Milliman or InterQual criteria.
  • Develop clinical recommendations and review eligibility of patients in preparation for hospital discharge huddle.
  • Assess prospective patients for eligibility, facilitate/coordinate transition from acute care setting, and prepare patient for discharge.
  • Perform utilization management functions, educate on program and eligibility criteria, and collaborate with respective site personnel regarding referrals and processes.

Requirements:

  • Bachelor's or Master's Degree in Nursing.
  • 2+ years of nursing/direct care experience in a healthcare setting.
  • Registered Nurse.
  • Experience in marketing/program coordination/program management/public health/home health.
  • Knowledge of Stark Laws.
  • Strong computer skills including SharePoint, Excel, EMR (electronic medical record).
  • Excellent organizational skills.
  • Active clinical license.
  • Valid Driver's License.
