We are seeking a highly skilled Palliative Care Nurse Practitioner Supervisor to join our team at ProTouch Staffing. As a key member of our team, you will be responsible for managing a cohort of participants with complex care needs, including frailty, multiple chronic illnesses, and end-of-life care.
Key Responsibilities:- Conduct thorough assessments of members identified as having complex and palliative care needs, including accurate history and physical assessment, medication reconciliation, goals of care identification, psychosocial/spiritual needs assessment, symptom management identification, and implementation of appropriate medical interventions.
- Determine and document appropriate follow-up plans in line with member goals and communicate these plans with members and caregivers.
- Complete Advance Care Planning documents.
- Review and include the most appropriate diagnosis codes, in addition to correct billing and procedure codes.
- Graduated from a nurse practitioner education program acceptable to NYSED or be certified as a nurse practitioner by a national certifying organization acceptable to NYSED.
- Minimum of two (2) years as a nurse practitioner in complex care and palliative/hospice care.
- A background in managed care, digital health, or startups is preferred.
- Active and unrestricted license to practice as a Nurse Practitioner in NY.
- Has Provider Transaction Access Number (PTAN).
- DEA Certification required.
- AANP or ANCC Board Certification required.
