MDS Coordinator

Mission Healthcare at Renton is seeking a highly skilled MDS Coordinator to join our post-acute care team. As a critical member of our interdisciplinary team, you will play a key role in reintegrating patients back into the community.
Key Responsibilities- Oversee scheduling of MDS assessments to ensure compliance with federal and state requirements.
- Review and certify completion of MDS assessments.
- Coordinate all aspects of the RAI process.
- Participate and/or conduct interdisciplinary team meetings.
- Transmit MDS assessments to secure data repositories as required by federal and state regulations.
- Registered Nurse (RN) with at least four years of LTC experience.
- Knowledge and expertise in the use of the Minimum Data Set (MDS) 3.0, RAI Manual, Resource Utilization Groups (RUGS), and CAAs.
- Knowledge of plans of care, discharge plans and required documentation for a nursing facility.
- Computer savvy, specific knowledge and use of Point Click Care, Rehab Optima and Microsoft Excel preferred.
- Ability to lead teams, provide education and collaboration with staff to ensure positive outcomes.
- RAC-CT Preferred.
- Detail-orientated with strong interaction with residents, families, supervisors and nursing personnel.
