Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Paramount Community Living and Rehab in Newton, Kansas. As an LPN, you will play a crucial role in delivering high-quality care to our residents.

Key Responsibilities:

  • Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
  • Monitoring and Assessment: Monitor residents' vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
  • Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident's needs.
  • Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
  • Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
  • Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
  • Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.

Requirements:

  • Licensure: Valid LPN license in KS
  • Education: Graduation from an accredited LPN program
  • Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred
  • Certifications: Current CPR/BLS certification required
  • Communications: Excellent verbal and written communication skills
