Responsibilities:
- Client Assessment and Care
- Conduct thorough assessments of clients' medical, physical, and psychosocial status to inform treatment plans.
- Develop and implement individualized treatment plans that address clients' unique needs and goals.
- Medication Management
- Prescribe and monitor medication as necessary to support clients' recovery.
- Collaborate with clients and other healthcare professionals to ensure safe and effective medication use.
- Collaboration and Communication
- Work closely with our multidisciplinary team to provide holistic care to clients.
- Communicate effectively with clients, families, and other healthcare professionals to ensure seamless care coordination.
- Documentation and Record-Keeping
- Maintain accurate and timely documentation of client observations, interventions, and progress in electronic health records.
- Ensure compliance with regulatory requirements and company policies.
- Professional Development
- Participate in ongoing education and training to stay current on best practices in addiction treatment.
- Engage in peer review and quality improvement initiatives to enhance clinical practice.
Requirements:
- Active Nurse Practitioner license in the state.
- 2 years of experience working with clients with substance abuse disorders preferred.
- Experience prescribing medication-assisted treatment (MAT) preferred.
- Strong communication and collaboration skills.
- Ability to work independently and as part of a team.
- Proficiency in electronic health records.
