Homecare Clinician

As a Homecare Clinician at MedStar Health, you will play a vital role in delivering high-quality patient care in the comfort of patients' own homes. Our team of dedicated healthcare professionals is committed to providing exceptional care and support to patients and their families.

• Provide direct and indirect patient care services under the supervision of a registered nurse.
• Develop and document individualized plans of care with specific care interventions and expected measurable patient outcomes.
• Evaluate progress toward goals and revise plans of care as needed.
• Establish, document, and implement personalized teaching plans for patients and their caregivers to help them achieve independence at home.
• Perform nursing procedures consistent with homecare protocols.
• Initiate and participate in regularly scheduled case conferences, which include all disciplines.
• Maintain necessary clinical records, collect data, and prepare reports on activities.
• Communicate regularly with the patient's physician, home healthcare team, and other key members of the care team.
